| (2) Does MolecularMD accept Medicare and Medicaid Insurance? |
| Yes. MoleculareMD is registered with Medicare and Medicaid and will bill on behalf of the patient. |
| (3) Does MolecularMD accept private health insurance coverage for the QRT-PCR BCR-ABL testing? |
| MolecularMD will submit claims for reimbursement with all private insurers as a non-registered healthcare provider. MolecularMD will provide the insurer with all necessary information to establish maximum reimbursement rates. Any remaining balance not covered by the insurance company will be billed to the patient by MolecularMD. To compensate for our current non-registered private insurance status, we are offering CML ALLIANCE patients special introductory pricing for the QRT-PCR BCR-ABL test.
